First Visit

When you come for your appointments, please remember to bring the following:

• Driver’s license or valid ID
• Insurance Card/Information
• Radiographs (x-rays), MRIs, CT scans or any other imaging studies (preferably on a disc)
• Reports for any previous imaging studies (reports alone are not sufficient – the images must be included)
• List of medications
• Referral letter
