Appointment Instructions:

When you come for your Appointments, please remember to bring the following:

  • Driver’s License or valid ID
  • Insurance Card/Information
  • Radiographs (x-rays), MRIs, CT scans or any other imaging studies (preferably on a disc)
  • Reports for any previous imaging studies (reports alone are not sufficient – the images must be included)
  • List of medications
  • Referral Letter

If you need radiographs (x-rays) prior to your appointment, please arrive 30-45 minutes prior to your appointment to allow you to register, fill out health forms, and get x-rays.

If you do not need radiographs prior to your appointment, please arrive 10-15 minutes early to register and fill out health forms.

You may also fill out, print, and bring the health forms with you prior to your appointment. Those forms may be found here.

Appointment Cancellation:

Please call the office during business hours (9 AM – 5 PM, Monday to Friday) to cancel an appointment. Please provide at least 1 day notice so that we may offer your appointment time to patients on the wait list.

Patients who do not comply with the cancellation policy may be subject to a cancellation fee.

We recognize that your time is valuable and we make every effort to run on time. Occasionally, emergencies occur or patients require more time than expected and these cause scheduling delays beyond our control. We apologize if we keep you waiting.
